As you respond to the following, keep in mind the Wege Foundation’s focus is West Michigan, particularly Grand Rapids. The five giving categories are, in rank order: Education, Environment, Arts & Culture, Health Care, and Human Services. Please take the Eligibility Quiz. If you are a good fit, our Grant Application will become available to you. Please complete the application, include a letter signed by a senior officer of your organization providing the following information. The more to the point and the less elaborate, the better your request. You may also include additional information, such as drawings, news articles, or relevant newsletters.

• Describe your organization, its history, and its mission.

• Summarize your proposed program or project and its specific goals.

• List a budget for your proposed program or project.

• Identify other anticipated sources of funding for your proposal.

• Describe how you will gage your project’s success and who will do that.

• Explain why this project is important to Grand Rapids at this time.

• Name other groups working with you on this project and what their roles are.

• Project a timeline.

• Include a copy of your IRS determination letter indicating your 501[C]3 status.

• List the names of those serving on your governing board.

• Describe your current staff, including volunteers.

• Provide a current annual operating budget, including expenses and revenue or an annual report.
